Responsibilities:
Team Select Home Care is looking for a Family Caregiver to join our Team Select family.
The Family Caregiver provides non-medical personal care and assists with activities of daily living, and related services in the home for sick and the disabled loved ones.
The Family Caregiver will report to the Director of Operation (DOO)/designee.

· Ensure quality and safe delivery of home health services
o Participates in development and implementation of client plans of care per home health care agency policy and procedure, as appropriate.
o Participates in client case conferences according to home health care agency policy and procedure, as appropriate.
o The provided home health aide services reflect client plans of care.
o Information regarding client plans of care is submitted to the Home Health Care Registered Nurse in a timely manner.
· Implements current Home Health Aide services
o Client plans of care are discussed with the Home Health Care Registered Nurse on regular basis.
o Client clinical records are documented per Home Health Care agency policy and procedure.
o Client assignments and reports are received from the Home Health Care Registered Nurse.
Qualification:
· Valid Driver’s License
· Auto Insurance (If applicable)
· CPR certification (In Home course)
· TB Screening
· Clean Background
