Lutheran Hospital of Fort Wayne, Indiana is looking for Registered Nurses (RN) Case Managers to join our team! Join our team of compassionate RN Case Managers, who serve as coordinator of an interdisciplinary approach to guide patients through the continuum of care with special focus on the clinical components of care/medical management and discharge planning to shorten length of stay while ensuring patient safety.
As patient advocate, Our RN Case Manager’s main focus is on Care Coordination and Discharge Planning.
Duties may include:
Collaboration with the physicians, patients, family members, and insurance companies to facilitate the achievement of desired outcomes.
Addressing ongoing needs, by maximizing patient, family and provider satisfaction
Optimizing resource utilization
Facilitating timely discharge
Assuring appropriate length of stay
Reducing unnecessary delays/other adverse variances to delivery of services
Assessing physician quality for peer review activities.
